Role Description
This is a full-time hybrid Nurse Case Manager role located in Southfield, MI, with some flexibility for remote work. The Nurse Case Manager will navigate healthcare systems, prevent medical issues, provide tailored treatments, and manage care complexities for patients with chronic conditions.
